Job Title: Nurse Care CoordinatorThe estimated salary for this role is between $23,400 and $28,200 per year.
Job Description:Key Responsibilities- Patient Assessment and Care Planning: Perform comprehensive patient assessments, including physical, psychological, and social factors. Develop, document, and modify patient care plans based on individual patient needs, ensuring personalized and holistic care.
- Clinical Care and Treatment: Administer prescribed medications and treatments safely, observing and reporting any side effects or adverse reactions. Conduct wound care, IV therapy, and other clinical procedures as needed, adhering to established standards and protocols. Monitor patient progress, record observations, and communicate significant changes to the healthcare team.
- Patient and Family Education: Educate patients and their families on diagnosis, treatment options, discharge instructions, and home care. Provide emotional support and reassurance, helping patients and families understand the care process.
- Interdisciplinary Collaboration: Work closely with physicians, specialists, and other healthcare professionals to ensure a seamless, coordinated approach to patient care. Actively participate in patient care team meetings, sharing insights and advocating for patient needs.
- Safety and Compliance: Adhere to all infection control protocols, including sterilization and sanitation procedures. Ensure that all patient interactions and care activities meet legal and regulatory standards, as well as hospital policies.
- Bachelor of Nursing Science (BSc) from an accredited institution.
- Valid Registered Nurse (RN) license in good standing with the appropriate regulatory body.
- Minimum of 5 years of clinical nursing experience.
- Prior experience in emergency care, pediatrics, or surgical care is an asset.
- In-depth knowledge of nursing protocols, healthcare regulations, and clinical practices.
- Excellent interpersonal and communication skills, with an empathetic approach to patient care.
- Strong analytical and critical thinking skills, with the ability to make swift, sound decisions in emergencies.
- High level of organizational skills and attention to detail.
- Ability to work effectively within a team and independently.
